Please arrive 15 minutes prior to your first appointment. Please print the following two documents and fill out accordingly prior to your initial appointment. Be sure to bring your Medicare ID card ( if insured with Medicare), form of payment for session and any pertinent medical information such as recent X-ray reports, MRI reports or CT scans to your appointment. All patients must complete the Medical History intake and HIPPA forms and sign these prior to treatment initiation. Patients out of network with insurance will ALSO need to sign the financial agreement.
Below are links to the submitting forms for private insurance. Medicare patients are excluded from this process. Welch PT will provide you the printed itemized list of treatment codes and receipt of payment required for submitting and reimbursement . Below are the most common local insurance providers ( Excellus, UHC). Welch PT is happy to assist you find other forms you may require and also will provide printed forms to you upon request.
