Physician referrals may be faxed at any time. Once a referral is received, you will be contacted directly to schedule an appointment within 24 hours with a phone call.

All patients may directly schedule an appointment without a referral ; however regular communication with your medical provider is of utmost importance, therefore referrals are encouraged. Sending a message on this form or emailing Dr. Welch directly at chelsea@welch-physical-therapy.com is the fastest way to obtain an initial appointment; however you may call the office phone to make an appointment directly, please leave a voicemail and you will be contacted within 24 hours.

If you anticipate surgery in the future, your surgeon will likely recommend scheduling your first post operative PT appointment for after surgery prior to the surgery to avoid having to wait weeks before your crucial first PT session with Dr. Welch.

When emailing for an appointment please include

• Full legal name
• Your Email address
• Date of Birth
• Reason(s) you are seeing PT services
• Your phone number
• Your primary care physicians (or other medical provider) name, phone number and fax number
• If you have Medicare, provide your Medicare number or a scanned Medicare ID card

Please print and fill out the two forms in the “before your first appointment” page to make your first appointment as streamlined as possible so less time is spent on paperwork and more time can be spent on what matters–YOU!
